Maximum Monthly Benefit Options:

• Members under age 50 may apply for up to $4,000 per month
• Members between ages 50 - 54 may apply for up to $3,000 per month

Waiting Period Options:

• 90 days
• 180 days

Maximum Benefit Period Options (as the result of an accident or sickness):

• 2 years
• 5 years
• 10 years
• To age 65

This insurance pays a benefit in the event an injury results in Death or Dismemberment or Loss of Sight within 365 days after the accident. For ‘loss of life’ or ‘loss of two or more limbs,’ the policy benefit would be the Principal Sum. For ‘loss of one limb,’ the policy benefit would be one half of the Principal Sum. You have the choice of selecting the Principal Sum amount that best suits your needs. Limits are available from $100,000 up to $500,000.
